Do I need to replace the radiator hose while flushing my radiator?
My car is a '94 & I believe the hose has never been replaced before or " don't fix it until it broke"?

how does the hose look like, is it deteriorating, does it have any leaks? if so, then replace, otherwise leave it till it breaks.

The upper and lower radiator hoses are fairly inexpensive ($9.80 & $14.79 respectively) and if they are still original I would suggest you replace them. I am of the preventive maintence mindset and would rather replace now then to experience a coolant flow out on the motorway and deal with that hassel and expense because I wanted to save a kings ransome of $24.59.
But thats just me. :)

just replace it. It might look ok but the inside of the hose might be thin.
